Insurance, Payment & Pricing
First Steps Fertility and Women’s Health is contracted with several commercial insurance plans and we’re happy to bill for services for in network claims.
Notably, we’re not contracted with United Health, Cigna, Molina or any Apple plans (this list is not exhaustive) but are happy to provide a superbill to be submitted for reimbursement.
Self-pay and out-of-network payment is due at the time of service.
Self-pay pricing for some common services:
- New patient consultation $225
- Monitoring ultrasound $180
- Lab processing fee for outside provider labs $25 (this fee is not billable to insurance and must be paid at the time of service)
- Saline sonohysterogram $525
- Hysterosalpingogram $700
- Partner IUI, includes wash, $500 (not billable to insurance unless prior authorization has been received)
- Donor IUI, frozen sperm only, includes thaw, $300 (not billable to insurance unless prior authorization has been received)
- Semen analysis $175
